Remember: If you are an eligible voter registering for the entire convention, you will automatically be registered to vote at the annual business meeting. Be sure to request an absentee ballot by Wednesday, Sept. 7, if you cannot attend the meeting by completing and emailing page two of this form to customercare@naphcc.org.
